Observation

Center reports the child was fed a documented allergen at snack causing the child's parent to need to administer the child's EPI pen. Staff reports the ingredients of the snack were not being properly reviewed prior to being served to the child. Center had all the proper documentation and paperwork onsite at time of the incident.
